Job Description
The Service Coordinator will play a crucial role in ensuring the provision of high-quality care to our clients. This includes coordinating centralized intake, developing service plans, and working closely with the Program Manager for Support Services.
Key Responsibilities:
- Adhere to AIDS Institute requirements for the provision of HIV Supportive Services program.
- Conduct intake assessment and screen clients' needs, work closely with the case manager and peer navigator to ensure implementation of service/care plan an/or case conference.
- Develops service plans that incorporate goals and objectives, frequency and duration of services, HIV-specific health and nutrition topics.
- Recruit and negotiate with vendors that will accept Apicha Food Voucher.
Required Skills and Qualifications:
- BA/BS in related field with two years of qualifying experience required. An advanced degree in one of the qualifying fields may be substituted for one year of qualifying experience.
- Experience working with target populations strongly preferred: HIV/AIDS individuals who are virally unsuppressed, individuals with Serious Mental Illness, serious substance use disorders (SUDs), and/or chronic hepatitis.
- Fluency in Spanish and/or Asian languages a plus.
- Excellent organization, written, verbal, and interpersonal communication skills in English required.
