We are one of the largest social service agencies in PA with positions available throughout the State with over 100 homes and over 900 employees in our IDD Residential Program. Our goal is to provide programs and support for more individuals with intellectual and developmental challenges, but we need more caring and compassionate teammates.

SCFF is currently seeking Program Specialist for our Residential IDD Program here in the Pittsburgh area. Annual Salary $48,000 and $52,500 depending on education and experience levels

  • A bachelor's degree with two (2) years of experience in the IDD field, or an associate's degree with four (4) years of IDD experience is required.
  • Knowledge of 6400 / 6500 regulations is a plus but not required.

RESPONSIBILITIES - THIS is NOT a full-time desk job.

  • This position requires to be out in the field at our locations conducting spot checks / quality reviews and providing immediate feedback.
  • This position also includes direct care and supervising Direct Care Staff - PCs / APCs / RSs
  • Skills needed : Strong Leadership, strong work ethic, attention to details, ability to create a team environment and encourage accountability and responsiveness
  • Oversight of up to 10 CLAs and 20-25 clients and their needs Provide full support to PCs - train, develop, coach / supervise PCs - set expectations, create a teaming environment and provide the support to drive success
  • Visit clients at home regularly : perform spot checks on client's needs (appts, medications, Physical Sites, Daily Logs) uncover areas of opportunities and brainstorm with the team on how to address issues, gaps. ensure best practices are uncovered and implemented to drive improvements compare hours worked vs hours budgeted to ensure we are in alignment with Company goals / KPIs - address any discrepancies Support team with Call Offs / Schedule Gaps Review / Approve Program Coordinators Timecards Conduct Weekly Calls with PCs - uncover best practices, uncover their pain points, work together to address them
  • Function with autonomy, diplomacy and professionalism within the guidelines established by this organization.
  • Promote the well-being of the human aspect of this organization and report any concerns to the Regional Director.
  • Function with the highest esteem in dealing with all employees, supervisors, consumer's stakeholders of this organization.
  • Be an advocate for all of the policies and protocols of the organization.
  • Observes all 6400 (Residential) regulations.
  • Documentation of On-Call progress notes, biweekly case notes, consumer assessments, purchase orders, census logs, fiscal forms, maintenance forms, residential change notification forms, quarterlies, IPP, medical forms, goal documentation, community life activities, lifetime medical history, monthly progress notes, and weekly progress note review.
  • Completion of all necessary DPW / HCFA annual requirements.
  • Attend department meetings, provider meetings, agency and committee meetings, staff and IC meetings, consumer and family team meetings, quarterly family meetings, Community Home Supervisor oversight, consumer quarterly meetings. Monitor overtime and hours worked.
  • Conduct orientation for new hires and employees transferring into new programs.
  • Participate in On-Call rotation as scheduled. Monthly site inspections and documentation for all licensed sites and unlicensed sites.
  • Schedule and attend (as required) psychiatric, medical, and therapy appointments as needed for consumers.
