POSITION STATEMENT :
The Case Manager is a bachelor's level paraprofessional with administrative and clinical experience providing assertive outreach, services linkage, benefits assistance, referral, and housing services for homeless adults with behavioral health problems. The case manager is responsible for facilitating program admissions and discharges, for monitoring client eligibility, and for ensuring that appropriate services / service linkages are provided to each person served. S / he is responsible for case planning, utilization management, data collection, and continual assessment to ensure that persons served are receiving the right level of care to meet their needs, and that services are tracked using the approved management information system.

Direct Duties :
MINIMUM EDUCATION AND EXPERIENCE QUALIFICATION REQUIREMENTS :
Bachelor's degree in psychology, counseling, human services, social work, nursing, rehabilitation or related field from an accredited college or university. Three years of clinical / administrative experience in providing supports to individuals with severe and persistent mental illness.
REQUIRED SKILLS :
CPR / First Aid Certification
Crisis Prevention and Intervention Training
Assistance with Self-Administration of Medication certification
Training in Defensive Driving
Training in Infection Control
Training in Suicide Risk Management
Training in Working with Persons with Serious Mental Illness
Training in PM-46 and how to identify and report abuse, neglect, mistreatment, and misappropriation of property
Training in 45 and 42 CFR, Rules and Regulations that safeguard a person's privacy
Training about Agency policies and procedures on both a daily and emergency basis
Training in Fire Safety and Evacuation
Training in Safe Food Handling
Training about local resources in the community
