Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Long-Term Care/Skilled Nursing Facility (LTC/SNF) team. This role offers an opportunity to provide compassionate care to a diverse resident population, including long-term care, skilled rehab, and dementia patients.

Key Responsibilities:

• Administer medications and treatments
• Blood glucose monitoring and insulin administration
• IV medication administration (Lasix, Bumex, antibiotics)
• TPN and tube feedings
• Wound care, including wound measurement
• Specimen collection and lab draws
• Foley catheter and trach care
• ADL assistance, assessments, and documentation
• Oxygen administration and management

Facility & Unit Details:

• Type of Facility: LTC/SNF
• Total Beds: 300 (divided into 6 units)
• Unit Assignments: May be assigned to various units based on staffing needs
• Charting System: PCC
• Nurse Staffing: RN/LPN mix with charge nurses and supervisors overseeing units
• No Respiratory Therapists On-Site (CPAP, BiPAP, O2, and trach management required)
• Pharmacy Available Monday–Friday (emergency drug closet access after hours)

Qualifications:

Experience: At least 1 year of LTC experience in the last 3 years
Licensure: Active LPN license
Certifications: BLS required
Prior PCC charting experience preferred

Schedule & Requirements:

• Shift Length: 8.25-hour shifts, including a 30-minute unpaid lunch break
• Weekend Requirement: Every other weekend (EOW)
• Scrubs: Any clean, professional scrubs
