APN Healthcare Solutions is urgently hiring multiple LVN/LPN for our client in Utica, NY for a 13 week contract with a high possibility of extension.

Job Description
We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to join our healthcare team. The LVN/LPN will provide direct patient care under the supervision of registered nurses and physicians, contributing to the delivery of high-quality healthcare services to our patients. The ideal candidate will demonstrate clinical proficiency, excellent communication skills, and a commitment to promoting the well-being and comfort of our patients.
Responsibilities:
- Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs according to established care plans and physician orders.
- Assist registered nurses and physicians in conducting patient assessments, implementing nursing interventions, and evaluating patient responses to treatment.
- Collaborate with interdisciplinary healthcare team members to develop and implement individualized care plans based on patient assessments, goals, and preferences.
- Document accurate and timely patient data, including assessments, interventions, medications administered, and response to treatment, in electronic medical records (EMR) or other designated systems.
- Educate patients and families about healthcare conditions, treatment options, self-care techniques, and medication management to promote optimal health outcomes.
- Perform routine procedures and nursing tasks, such as wound care, catheterization, tracheostomy care, and tube feedings, in accordance with established protocols and standards of practice.
- Monitor patients for changes in condition, symptoms of complications, and signs of distress, and promptly report concerns to the registered nurse or physician for further evaluation and intervention.
- Maintain a safe and therapeutic environment for patients by adhering to infection control protocols, safety regulations, and facility policies and procedures.
- Assist with the coordination of patient care activities, including scheduling appointments, arranging diagnostic tests, and facilitating referrals to specialists or other healthcare providers.
- Participate in interdisciplinary meetings, staff development programs, and quality improvement initiatives to enhance clinical knowledge, skills, and performance.
