What you will do in this role:
- Provides treatment and documentation for residents assigned to caseload
- Provides appropriate physical therapy interventions based off of resident evaluation; collaborates cooperatively with other patient team personnel in maintaining standards for professional physical therapy practice and interdisciplinary teamwork.
- Observes and assesses treatment affect, accomplishment of short- and long-term goals. Recommends change to Physical Therapist, if needed
- Performs an appropriate screening on each patient as per policy. Performs accurate and timely GG scoring. The population is adult and geriatric residents in an SNF setting.
- Able to assess resident pain interfering with optimal level of function of participation in rehabilitation; makes appropriate interventions to reduce pain; makes appropriate physician contact for intervention.
- Follows resident specific plan of care for physical therapy.
- Other duties as assigned.
Qualifications
- Current Florida professional licensure as a Physical Therapy Assistant
- Must have long term care experience. Knowledge of PDPM, Section GG and LTC program development.
- Familiar with Net Health and Point Click Care
Salary range: up to $38.00 per hour. We would expect those hired for the job to have a salary within his hourly rate, however, certain circumstances may allow us to pay outside of this hourly rate.
