What are the responsibilities and job description for the MDS COORDINATOR position at Apostolic Christian Skylines?
Scope of Position:
The objective of MDS coordinators is to promote the physical and emotional wellbeing of nursing facility residents. They use a Resident Assessment Instrument (RAI) to gather information from residents and their families during initial and periodic interviews. Support and promote the vision, mission, and policies of AC Skylines.
Job Responsibilities:
- Completes MDS calendar. Prepares electronic transmissions and sends to state. Obtains confirmation and validation. Sets out weekly tracking papers. Audits tracking and in-services staff on completing tacking sheets. Reviews tracking and completes annuals, significant change, quarterly MDS assessments, Medicare MDS assessments, and admission MDS assessments. Prints MDS and obtains needed signatures on forms.
- Attends Care Plan Conferences.
- Communicates with families as requested by resident.
- Tracks Medicare Part A days and RUGS.
- Assist with auditing charts especially related to MDS, Care Plans, and Assessment Sheets.
- Sets out behavior tracking for residents and collects them when necessary.
- Does tracking of vision, hearing, skin, falls, restraints, Psychotropic, and bowel and adds diagnosis and allergies to resident face sheet in computer. Prioritizes diagnosis for billing purposes.
- Completes all discharges and reentry tracking for the state.
- Assists with tour during state survey.
- Attends weekly Medicare meetings, reports on Medicare residents and other concerns.
- Works as nurse on floor when needed.
- May be assigned for extra duties by administrator or DON.
- Assists with resident screening and admissions as necessary.
- Completes necessary paperwork needed to process Long Term Car Insurance policies. Works with residents and/or families through this process.
- Completes Medicaid Census information for oxygen billing and communicates with Accent Health Care.
- Complete Corporate Compliance training annually as required.

Qualifications: The MDS Coordinator needs to have a current RN nursing license in the state of Illinois and have or be willing to obtain AANAC MDS Certification. Personality requirements include but are not limited to being a team player, possessing empathy, displaying work that reflects accuracy and consistency, effective communication skills, and compassion for the elderly.
