Scope of Position: The primary purpose of the Memory Care Activity Coordinator is to plan, schedule, and direct individualized and group activities based on the interest of residents. Work with the interdisciplinary team and assist developing plan of care that addresses resident activity needs. Complete activity resident assessments within timelines as outlines by state regulations. Perform other related duties as directed by the activity director.
Job Responsibilities:
- Plan, schedule and implement a program of individual and group activities based on residents’ needs in the Memory Care Unit.
- Complete activity Nursing Center resident assessments within timelines as outlined by state and federal regulations. Specifically, the MDS section F and complete in Matrix.
- Identify and document in the resident’s care plan the activity interests, needs, and problems of the resident.
- Document all interaction with residents, staff and visitors in the assessment and care plan as required by state standards.
- Act as an active member of the interdisciplinary team and is required to attend all Memory Care resident care conferences or assign someone to do so. Work with Care plan team to develop a plan of care that addresses the activity needs of the residents.
- Plan and implement evening and weekend functions as necessary.
- Take part in in-service education and attend all in-service sessions relating to the Activity Department.
- Plan, schedule and implement room visits and in-room activities for residents unable to leave their rooms.
- Maintain an activity attendance record for each resident.
- Maintain an adequate supply of materials to implement activity programs.
- Recruit, schedule and supervise activity personnel.
- Conduct timely, formal Job Performance reviews with Salem activity personnel.
- Follow Resident’s Rights policies at all times.
- Accept assigned duties in a cooperative manner.
- Assume accountability for data contained in the employees’ handbook.
- Observe all facility safety policies and procedures.
- Perform other related duties as directed by the Executive Director.
- Complete Corporate Compliance training annually as required.

Requirements:Qualifications:
- High School Diploma minimum
- Recreational Therapy preferred
- 36 hour course completed within 3 months of hire
