POSITION REQUIREMENTS : Graduate of an accredited school of Physical Therapist Assistant.
Licensed to practice Physical Therapy by the Virginia Board of Physical Therapy. Meets minimum continuing education requirements. Must have a valid driver’s license and willingness to travel to multiple counties. Preferred two plus years of Home Health experience and working with the frail and elderly.
Demonstrate clinical competency with geriatric population. Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact. Must undergo criminal background check and drug screening upon hire.
SUMMARY : To assist in providing quality physical therapy care while maintaining positive level of interaction with facilities and patients under the direction of licensed personnel. Screens and reassesses patients with regard to level of mobility, independence and overall level of care. Implements and executes treatment of all patients who could benefit from intervention as prescribed by PACE provider and outlined by physical therapist.
DUTIES AND RESPONSIBILITIES :
- Provides patient care based upon therapist evaluation and plan of care and takes into consideration the patient’s age and level of development when implementing treatment programs and assisting with changes in treatment goals.
- Carry out physical therapy interventions in the following settings including, but not limited to : PACE Center. Alternative Care Sites, Assisted Living Facilities and in their home :
- Coordinates patient care and clinical activities through written and / or verbal communication with patients, families, therapists, managers, and members of PACE Interdisciplinary Team.
- Documents in the patient’s medical record and completes other patient care-related forms in compliance with regulatory agencies and department / hospital policies and procedures.
- Supervises and collaborates with support staff, volunteers and students to achieve efficient clinical activity and quality patient care.
- Participates in other patient care and department / professional activities as requested / assigned including but not limited to presenting / attending in services, performance improvement, program development / evaluation, new employee orientation, committee membership, peer review and attending staff meetings.
