What are the responsibilities and job description for the Service Coordinator position at Appalachian Community Health Center?
Appalachian Community Health Center is looking to fill two full-time positions, a Service Coordinator at our main office location in Elkins and an Admissions & Crisis position in our satellite office in Buckhannon. These positions require a minimum of a bachelor's degree in a human services field, though non-traditional bachelor degrees may be considered with a history or classes appropriate within the field. Both positions offer an excellent opportunity to gain vital experience in the behavioral health field. You will have the opportunity to work closely with and learn from other staff at various levels to ensure the Center's clients' needs are being met with consistency and compassion. On-the-job training for the positions as well as other required training are provided by the Center.
A Service Coordinator provides services to existing clients such as linking, referrals, advocacy, and participating in the treatment plan process to consumers of all ages with mental and behavioral health needs. This position has a set schedule of Monday through Friday, 81m - 5pm with an hour lunch break.
The Admissions and Crisis position has the primary responsibilities of scheduling and completing intakes with new or re-enrolling consumers as well as providing crisis services as needed. This position requires regular office hours as well as some participation in the on-call rotation. While primarily working from the Upshur office, there will be some expected travel within our service area of Upshur, Barbour, Randolph and Tucker counties, as needed.
