Appalachian Community Services (ACS) an ncgCARE partner, provides mental health, substance abuse and intellectual/developmental disability services to individuals and families within the seven western counties of North Carolina. These services include counseling and psychiatry, substance use treatment, mobile and residential crisis stabilization, intensive in-home, community support teams, peer support and group homes.
Job Duties:
- Coordinate and conduct the assessment and reassessment of clients' clinical needs, including the functional needs assessment to determine service scope and anticipated outcomes.
- Assign team members with specialized clinical expertise as needed for each client.
- Provide individual and family therapy to each client assigned to the Enhanced Service team.
- Ensure that services are delivered according to the client’s PCP and in alignment with medical necessity criteria.
- Monitor and evaluate services, interventions, and activities provided by the team.
- Provide clinical supervision to all team members, ensuring adherence to individual supervision plans.
- Ensure fidelity to the service definition and that clients are seen as needed to meet their clinical requirements.
- Review and approve service documentation, including service notes and PCPs, to ensure accuracy and compliance with regulatory standards and that the documentation is completed on time.
- Conduct regular team meetings to review case progress, discuss interventions, and address challenges.
- Travel within the service area and work flexible hours as needed.
- Facilitate crisis response and linkage to additional services as needed, ensuring comprehensive care for clients.
