Appalachian Community Services (ACS) an ncgCARE partner, provides mental health, substance abuse and intellectual/developmental disability services to individuals and families within the seven western counties of North Carolina. Our mission is to promote the emotional and physical well-being of consumers in a safe and respectful environment.

We are committed to providing a wide array of services to effectively meet the changing needs within the community. These services include counseling and psychiatry, substance use treatment, mobile and residential crisis stabilization, intensive in-home, community support teams, peer support and group homes.

Job Duties:

  • Coordinate and conduct the assessment and reassessment of clients' clinical needs, including the functional needs assessment to determine service scope and anticipated outcomes.
  • Assign team members with specialized clinical expertise as needed for each client.
  • Provide individual and family therapy to each client assigned to the Enhanced Service team.
  • Ensure that services are delivered according to the client’s PCP and in alignment with medical necessity criteria.
  • Monitor and evaluate services, interventions, and activities provided by the team.
  • Provide clinical supervision to all team members, ensuring adherence to individual supervision plans.
  • Ensure fidelity to the service definition and that clients are seen as needed to meet their clinical requirements.
  • Review and approve service documentation, including service notes and PCPs, to ensure accuracy and compliance with regulatory standards and that the documentation is completed on time.
  • Conduct regular team meetings to review case progress, discuss interventions, and address challenges.
  • Travel within the service area and work flexible hours as needed.
  • Facilitate crisis response and linkage to additional services as needed, ensuring comprehensive care for clients.
