Appalachian Hospice is looking for full-time RNs to serve patients in Cherokee, Bartow and Pickens counties. Appalachian Hospice based in Jasper, GA (with offices in Blairsville, Calhoun and Cleveland, GA) is a locally owned and operated company. Its owners have successfully operated other Georgia based hospice and home health care providers. Their proven record is founded on putting its patient and employees first. Overview:
The Hospice Nurse - RN (Registered Nurse) utilizes a Case Management approach to facilitate comprehensive nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary plan of care and regulatory requirements. In this role, you will function as primary liaison between physician, patient/family and hospice team. Further, you will
- Provide direct patient care services, with a reasonable case load, to hospice patients and supportive care to the patient’s family
- Manage an interdisciplinary group consisting of LPNs, CNAs and support staff to enhance the quality of hospice care
- Evaluate patient/family needs and assist in the preparation and execution of the plan of care for each patient
- Maintain all assigned patient clinical records
- Participate in company quality assessment and performance improvement programs
Qualifications:
- RN (Registered Nurse) license in Georgia.
- Current CPR certification.
- Previous experience in hospice or home health
- Willingness to serve weekday on-call.
- Must have reliable automobile, valid driver's license, and the minimum state required liability auto insurance.
