Description
The Licensed Integrated Behavioral Health Provider Float is responsible for providing outpatient mental health counseling for patients within the primary care setting, and school settings, for multiple Appalachian Mountain Health locations. The Integrated Behavioral Health Float provider will work collaboratively with an interdisciplinary team of providers, including, primary care MD’s, NP’s, PA’s, Nurses, clinical pharmacists, medical assistants, psychiatric providers, educators, and school administrators at multiple sites. Responsibilities of this role include short term mental health counseling to Appalachian Mountain Health patients to help them build resiliency and internal resources to cope with stressors in their lives. This role will support AMH clinics within local schools and requires a provider with experience working with all ages – early childhood into adulthood.
Drug Screen and Background Check Required.
Responsibilities
Work collaboratively within a team of medical and mental health providers.
Flexibility to work within multiple clinics/sites within each week.
Maintain open communication and accessibility to encourage mental health integration in the clinic setting and with community partners.
Interviews individuals to assess needs and develop appropriate response plans for treatment.
Performs Bio-psychosocial assessments for all ages.
Work with a variety of patients with a wide range of mental health diagnoses, and lived experiences, including substance use disorders.
Provides information and referrals for counseling and service acquisition as appropriate to the assessed need.
Understands rules and regulations as it applies to both clinical mental health counseling, health care, and educational institutions.
Provides consultation services for mental health recommendations to patient’s treatment team.
Initiates treatment interventions, e.g., individual, school, and family counseling and crisis interventions as appropriate.
Support patients through difficult circumstances with an emphasis on building sense of internal locus of control.
Document all patient interaction and referrals.
Performs patient education.
Charts and maintains notes within 36 hours in compliance with CMS documentation standards.
Protects confidentiality and the abide by HIPPA Laws as well as 42 CFR.
