Community Health Advanced Practice Provider
The primary purpose of this role is to provide comprehensive clinical care to underserved populations in remote areas via a mobile health unit. The APP will play a crucial role in improving access to healthcare services by delivering primary care, managing chronic diseases, providing preventive care, and offering health education to individuals and families within this rural community.

- Understanding of the challenges and healthcare disparities facing Eastern Kentucky residents and the ability to offer care that is culturally appropriate and patient-centered. o Provide primary care services, including assessment, diagnosis, treatment, and management of acute and chronic conditions (e.g., hypertension, diabetes, asthma, etc.).
o Conduct physical examinations, order and interpret diagnostic tests, and prescribe medications as appropriate.
o Provide preventative care, including screenings (e.g., cancer, blood pressure, cholesterol), immunizations, and health counseling.
o Offer maternal and child health services, including prenatal care, well-child visits, and adolescent health screenings.
o Collaborate with medical staff, including physicians, specialists, and other healthcare providers, to develop and implement individualized treatment plans.
o Educate patients and families on disease prevention, healthy lifestyle choices, and medication adherence.
* Mobile Health Unit Operations:
o Operate as part of a mobile health team, traveling to designated locations within the service area in the Eastern Kentucky region.
o Ensure proper documentation of patient visits, including history, examination findings, treatment plans, and follow-up care.
o Participate in community outreach and engagement activities to encourage local residents to access services offered on the mobile health unit.
o Maintain and manage medical supplies, equipment, and patient records in compliance with regulatory standards.
- Health Education and Advocacy:
o Promote health literacy by providing culturally sensitive health education, including topics such as chronic disease management, mental health awareness, and substance abuse prevention.
o Advocate for health policies and resources that address the unique needs of the community.
* Collaboration and Reporting:
o Work closely with local healthcare facilities, public health agencies, and community organizations and area businesses to coordinate care and services.
o Maintain accurate, up-to-date records in accordance with healthcare regulations and standards, including privacy laws such as HIPAA.
o Report on patient outcomes, community health trends, and other relevant data to program directors and health leadership.
