Purpose of the Clinical Care Coordinator: Assure that all aspects of the resident’s personal care and service plan are implemented while assuring that the resident is treated with dignity and respect, assisting each resident to be as independent and self-sufficient as possible.
Qualifications: Must be a Licensed Practical Nurse in the state of Michigan; Proof of training in TB testing; Proof of training and competency in medication administration; Leadership ability. Must be able to physically and mentally perform essential functions of this position. Ability to communicate effectively in English. Must be able to communicate with a variety of personality types. Able to effectively supervise and support staff in providing direct service to residents; communicate with family members, physicians, labs, pharmacy, and ancillary personnel. Must demonstrate compassion towards residents. Must be able to pass the state mandated criminal background screening and random drug testing.
Essential Functions:
- Treat all residents with dignity and respect
- Provide excellent nursing care and service according to individual care plans.
- Monitor lab orders and follow up on results.
- Assist with Med Tech training, documentation, and counselings/coaching for errors to uphold nursing standards.
- Communicate with physicians and nursing staff regarding resident care needs and review pharmacy logs and monitor med rooms for cleanliness and accuracy.
- Uphold Resident Rights and comply with infection control protocols.
- Track and administer resident TB skin tests and B-12 injections, and assure that we are keeping up to date tracking on expired medications/injections.
- Participate in on-call duties and Manager on Duty rotation.
General Responsibilities:
- Serve as a goodwill ambassador for the living center.
- Complete required documentation and participate in training opportunities.
- Report safety hazards and respond to resident requests promptly.
- Assist the RSC and Resource Manager with annual and 90 day reviews.
- Comply with rules and regulations governing Homes for the Aged Work cooperatively with others on the same shift and across shifts
- Report to work as scheduled respond to resident requests for assistance, including “call lights"
Physical & Mental Requirements:
- Able to lift 50 lbs and assist residents with personal care, such as toileting, showering, getting dressed, etc. This may include needing to stand, bend, twist, turn, stoop, squat, sit, kneel, pull, push, and walk long distances on hard surfaces, for extended periods of time.
- Strong communication skills to interact with residents, nurses, staff, and management.
- Emotional stability to manage various resident care needs.
