PATIENT CARE COORDINATOR

This is a liaison/sales position that requires a motivated self-starter with great networking skills, sales and community relations knowledge, understanding of the local area, and dependability/integrity.
PATIENT CARE COORDINATOR BENEFITS

PATIENT CARE COORDINATOR ESSENTIAL FUNCTIONS
-Do 1:1 contacts, occasional group presentations, and occasional community events or trade shows to develop referral relationships.
-Meet with families to educate about home health and hospice services.
-Track accounts and strategize how best to gain their confidence.
-Assist staff with getting physician signatures for orders where needed.
-Liaison with hospitals, physician offices, skilled nursing and assisted living facilities.
PATIENT CARE COORDINATOR QUALIFICATIONS
-Must have experience in home health and hospice or related field.
-Community relations work preferred.
-Hospital/Physician marketing experience a plus.
-Clinical license is a plus, but not required.
-Require a valid Utah driver's license.
-Must have a dependable automobile.
