**Job Title: Licensed Practical Nurse (LPN) – Intellectual and Developmental Disabilities (IDD)**

**Location:** Atlanta, GA

**Work Schedule:** Up to 11 hours per day

**Job Type:** Full-time/Part-time (depending on availability)

**Job Description:**

We are seeking compassionate, skilled, and dedicated Licensed Practical Nurses (LPNs) to provide exceptional care for individuals with Intellectual and Developmental Disabilities (IDD) in the comfort of their homes in the Atlanta area. This role offers the opportunity to work closely with clients, ensuring they receive the highest quality of care in a supportive, home-based environment. The ideal candidate will have experience with IDD clients or a strong interest in providing compassionate care to individuals with special needs.

**Key Responsibilities:**

- **Provide Direct Care:** Deliver skilled nursing care to individuals with Intellectual and Developmental Disabilities (IDD) in their home. This includes assisting with daily living activities, administering medications, and supporting the client’s physical and emotional well-being.

- **Health Monitoring:** Regularly assess and document the client’s health status, including vital signs, physical condition, and any changes in behavior. Report any significant changes in health to the supervising Registered Nurse (RN) or physician.

- **Assist with Medications and Treatments:** Administer prescribed medications and treatments according to care plans. Ensure accurate documentation and monitor for side effects or adverse reactions.

- **Implement Care Plans:** Follow individualized care plans created by the RN, ensuring that all aspects of care are carried out as per the plan. Adjust care as needed, in consultation with the RN or other healthcare team members.

- **Collaboration:** Work closely with other healthcare professionals, including RNs, physicians, and family members, to ensure comprehensive care. Participate in team meetings to discuss the client’s progress, needs, and any adjustments to the care plan.

- **Client Advocacy:** Act as an advocate for the client’s needs, ensuring that they receive the highest quality care and that their rights and preferences are respected.

- **Support Daily Living Activities:** Assist clients with mobility, personal hygiene, feeding, and other daily living activities as necessary. Promote independence while providing support when needed.

- **Emergency Response:** Be prepared to respond to medical emergencies, provide first aid, and ensure the client’s safety at all times. Notify the RN or physician if there are any concerns or issues that require further intervention.

- **Family Education and Support:** Provide education to family members about the care plan, medication regimen, and how to support the client in managing their condition. Offer guidance on daily care needs and maintaining a safe home environment.

- **Maintain Accurate Records:** Document all care provided in compliance with healthcare regulations and organizational policies. Maintain confidentiality and adhere to HIPAA standards.

- **Safety and Cleanliness:** Ensure that the client’s home environment remains safe and clean, following infection control practices and maintaining a hygienic space for care.

**Qualifications:**

- Valid Licensed Practical Nurse (LPN) license in the state of Georgia.

- At least 1 year of experience in nursing, preferably with experience in caring for individuals with Intellectual and Developmental Disabilities (IDD).

- Basic knowledge of medical terminology and nursing procedures related to IDD care.

- Strong ability to assess client needs and report changes in health status.

- Compassionate and patient demeanor, with a genuine passion for working with individuals with disabilities.

- Excellent communication and interpersonal skills to interact effectively with clients, families, and healthcare teams.

- Ability to work independently, following established care plans while keeping the RN or healthcare team informed of any issues.

- Ability to lift, assist with mobility, and provide physical care as needed.

- Current CPR and First Aid certification (or willingness to obtain).

- Strong organizational and time management skills.

**Preferred Qualifications:**

- Experience working with individuals with Intellectual and Developmental Disabilities or in a similar home health or residential care setting.

- Ability to work collaboratively as part of a multidisciplinary team, including physicians, RNs, and family members.

- Knowledge of client care plans and health monitoring procedures specific to IDD care.

- Familiarity with HIPAA regulations and maintaining client confidentiality.

**Physical Requirements:**

- Ability to stand, walk, bend, and lift clients as needed throughout the shift.

- Comfort with providing care in the home environment, which may include assisting with mobility, lifting, or transferring clients.

- Ability to adapt to the physical demands of the job, including assisting clients with daily activities.
