The Client Care Coordinator will coordinate and manage the care of clients requiring services across all Aprendamos Family of Services Programs. The Care Coordinator will serve as a central point of contact for clients and their families, ensuring they are informed about and involved in all care planning processes. The Care Coordinator will ensure effective communication and collaboration among disciplines to provide comprehensive and integrated care.
KEY ACCOUNTABILITIES:
- Providing collaborative care identified by the initial biopsychosocial assessment tool.
- Meeting client situations by setting measurable goals with timelines.
- Meeting cross program documentation guidelines as needed.
- Meeting the EI program billing guidelines as needed.
REQUIRED QUALIFICATIONS:
- Bachelors in Social Work or early childhood education preferred.
- Knowledge of local, state and federal support services
ESSENTIAL JOB FUNCTIONS:
- Interview and identify the family's current needs and barriers.
- Develop a measurable plan with the client and family to address barriers.
- Facilitate communication and collaboration among BCBAs, SLP’s, OT’s, PT’s, and other disciplines.
- Schedule multi-disciplinary team meetings to discuss client progress, treatment plans, and goals.
- Assist the family with accessing needed community and state resources as
- needed.
- Track goal progress quarterly and update the multi-disciplinary team.
- Complete quarterly check ins with families currently on the Mariposa Autism Service Center waiting list to provide resources as needed.
- Providing the families in EI with a warm introduction to MASC services when their child is diagnosed with Autism.
- Data collection on all of the shared families within AIT Family of Services.
- Connecting families to the Autism Diagnostic partners if their child has not received a diagnosis (child has already aged out of EI and is receiving services from Esperanza, Carino, Alegria).
OTHER FUNCTIONS AND RESPONSIBILITIES:
- Software and computer proficiency to document the services provided to children and their families.
- Demonstrate the organizational skills needed to complete the job duties of the position.

- Perform other position-related duties as assigned.
PREFERRED QUALIFICATIONS/SKILLS:
- Bilingual Preferred.
- Must have a working knowledge of social theories and practices.
- Ability to use decision-making skills that objectively and fairly evaluate a situation, ensuring an ethical decision is made that balances the needs of the client, the needs of the oneself, and the needs of the company.
- Ability to work in a changing environment, gathering relevant information and applying critical thinking to integrate a change or modify a program.
Job Type: Full-time
Pay: $45,000.00 - $55,000.00 per year
