Position Overview
The Program Supervisor, CalAIM Implementation is responsible for the successful delivery and operational oversight of APF’s CalAIM initiatives, including Enhanced Care Management (ECM), Community Supports, and related services. This leadership role ensures program fidelity, supports a team of frontline staff, and fosters strong relationships with Managed Care Plans and community-based partners. The ideal candidate brings a deep understanding of Medi-Cal transformation, a commitment to health equity, and a passion for empowering communities through integrated, person-centered care.

The Pay Range For This Role Is
75,000 - 87,500 USD per year(Los Angeles County, CA)

Position Overview
The Program Supervisor, CalAIM Implementation is responsible for the successful delivery and operational oversight of APF’s CalAIM initiatives, including Enhanced Care Management (ECM), Community Supports, and related services. This leadership role ensures program fidelity, supports a team of frontline staff, and fosters strong relationships with Managed Care Plans and community-based partners. The ideal candidate brings a deep understanding of Medi-Cal transformation, a commitment to health equity, and a passion for empowering communities through integrated, person-centered care.
Key Responsibilities
- Supervise day-to-day CalAIM operations, including staffing, workflows, and service quality
- Monitor key performance metrics and client outcomes to ensure program excellence and innovation
- Provide coaching and training to ECM Case Managers and Community Health Workers on trauma-informed care, documentation standards, and culturally responsive practices
- Build and maintain partnerships with Medi-Cal Managed Care Plans and local stakeholders to ensure coordination of care, smooth referrals, and service resolution
- Oversee data collection, reporting, and quality assurance aligned with CalAIM guidelines
- Adapt internal workflows in response to evolving state policies and regulatory guidance
- Participate in community engagement efforts and collaborate with partners to identify emerging needs and expand access to integrated care
Required
- Bachelor’s degree in Social Work, Public Health, Nursing, or related field (Master’s preferred)
- 3 years of supervisory experience in a health, behavioral health, or social service setting
- Experience implementing CalAIM or similar Medicaid transformation programs
- Knowledge of Enhanced Care Management, Community Supports, and Medi-Cal managed care
- Strong leadership, communication, and organizational skills
- Proficiency in case management and reporting systems
- Bilingual (preferred)
- Deep passion for equity, integrated care, and community-driven solutions
- Familiarity with health and housing systems in Los Angeles and Orange Counties
- Experience with trauma-informed and culturally responsive care approaches
