Compensation and Schedule:
- Hourly Position: Full-time (40 hours per week)
- Work Schedule: Monday-Friday, between the hours of 8 AM 4 PM, with flexibility in scheduling

Responsibilities:
As the Community Inclusion Services Lead, youll play a key role in developing, implementing, and evaluating services that improve the functional abilities of individuals in our program, empowering them to independently engage in their communities. Your work will include:
- Community Engagement: Collaborate with funders and community partners to develop the Community Connections service line, focusing on individualized, person-centered services.
- Activity Coordination: Facilitate group activities for community-based day services and ensure the necessary resources and tools are available for successful service delivery.
- Data and Reporting: Maintain detailed records to evaluate service effectiveness, including daily charting and required assessments.
- Travel and Advocacy: Plan and advocate for extended trips and events that enhance community participation and inclusion for the individuals served.
- Continuous Improvement: Ensure high standards of service delivery by attending team meetings, participating in required trainings, and maintaining up-to-date documentation and case files.
Qualifications:
Required:
- At least one year of documented experience working with individuals with disabilities
- Availability Monday-Friday, 8 AM 4 PM, with flexibility to work 40 hours/week
- Access to reliable, non-public transportation, with the necessary insurance coverage for daily use
- Strong verbal and written communication skills
- Proficiency in Microsoft Office (2010 and above), Google accounts/applications, and basic computer operations
- Excellent organizational and program planning skills
- Well-connected within the community
- Ability to work independently and as part of a team
Preferred:
- Bachelors degree in Therapeutic Recreation or a similar field
- Experience with assessments and program evaluation
- Scheduling experience
- Knowledge of proactive behavioral approaches and precision teaching techniques
