Job Overview
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care and providing guidance to individuals with complex medical needs.
Responsibilities
- Collaborate with healthcare professionals to develop and implement patient care plans
- Monitor and assess patient progress and make necessary adjustments to the care plan
- Provide education and support to patients and their families on managing health conditions
- Coordinate referrals to specialists and other healthcare services as needed
- Conduct assessments and evaluations to ensure quality care delivery
- Maintain accurate and up-to-date patient records
Experience
- Previous experience in nursing, particularly in Pediatrics, ER, Medical-surgical, or ICU settings
- Proficiency in patient care, including vital signs monitoring and medication administration
- Knowledge of primary care practices and procedures
- Ability to work effectively in a fast-paced hospital environment
- Experience with inhalation therapy is a plus
