Position Description:Contractor will ensure that Contract LPN provides skilled nursing care to patients in Acute Care, Primary Care and Specialty Clinics in Charleston and VA Clinics (CBOC’s). He/she may perform those approved Medical Center procedures that are described in the Licensed Practical Nurse Act, Vocational Nurse VA Qualification Standards. The LPN demonstrates increasingly more independence in his/her management of patient care and demonstrates competence in caring for adult psychiatric, medical, and geriatric patients. Under the supervision of the nurse manager, the LPN works closely with others as needed in the Specialty Clinics and Primary Care setting throughout the Ralph H. Johnson VAMC and VA Clinics (CBOC’s). The LPN functions both dependently and independently with guidance and direction from the Nurse Manager/designee.
Work Schedule: Monday through Friday, no holidays no weekends. 8.5-hour shifts start no earlier than 0700 and end no later than 1730.
Primary Responsibilities
- LPN works closely with others as needed in the Acute Care Medical/Surgical areas and Primary Care setting throughout the Charleston VAMC surrounding Community-Based Outpatient Clinics. The LPN functions both dependently and independently with guidance and direction from the Nurse Manager/designee. LPN must have active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
- Knowledge and skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures. Observation and documentation will include patient's response to medication administered and the reporting of any noted change in patient's condition to RN or MD/DO.
- Demonstrated ability to serve as a responsible member of the nursing team and interact [in an appropriate manner] with patients, family members, professional and [other] supportive personnel [involved in the delivery of] patient care, incorporating acceptable, established customer service standards into practice.
- Charleston VAMC Mental Health Psychiatric unit can expect to work 12-hour shifts, with alternating weekend shifts preferred. Charleston VAMC Primary Care/Specialty Clinics unit can expect to work 8-10-hour shifts.
Minimum Qualifications
- Minimum Qualifications
- Active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
- Graduate of a state approved school of practical/vocational nursing.
- A current Cardiopulmonary Resuscitation (CPR)/Basic Life Support (BLS) certificate accredited by the American Heart Association.
- Med/Surg - minimum 2 years current experience (must have worked within the last six months) in a medical/surgical unit.
- Long Term Care/Nursing Home - minimum 2 years current experience and (must have worked within the last six months) in a long-term care setting.
- Psychiatric/Mental Health - Minimum 2 years current experience (must have worked within the last six months) experience in an inpatient psychiatric unit.
- Primary Care/Specialty Clinics - Minimum 2 years current experience and (must have worked within the last six months) in a primary care setting.
