Job Summary
Job Title: Long-Term Care Nurse
Location: Miami VA Healthcare System (including 7 Community-Based Outpatient Clinics)
Period of Performance: 03/12/2025 – 03/11/2030
Shift Schedule: Rotating eight (8), ten (10) and twelve-hour (12) tours on rotating day, night, and evening shifts, to include weekdays, weekends, and holidays.
Position Description:
The Long-Term Care Registered Nurse (RN) delivers comprehensive nursing care to residents across three long-term care units, including two 45-bed units for long-term and short-stay residents, and one 10-bed unit providing hospice and palliative care services. The RN is responsible for providing individualized, age-specific care that focuses on enhancing quality of life, promoting comfort, and supporting residents' health needs throughout their stay. This role requires strong clinical skills, compassionate care delivery, and effective communication to meet the unique needs of long-term care residents and their families in a supportive and patient-centered environment.

Key Responsibilities:
Unit Description: The long-term care division is comprised of three (3) long-term care units of which two (2) are 45 beds with both long-term and short-stay residents, and one (1) with ten (10) beds that provide hospice and palliative care.
Skills: The Long-Term Care registered nurse is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age- specific components. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, provides family education. The long-term care nurse shall be responsible for patient ADLs (activities of daily living), assessments, BCMA medication and procedures per established policies and guidelines (i.e.). Barcode medication and administration, IV therapy, blood transfusion, monitor blood pressure, weight and hydration, administration tube feeding, management of behavioral patients, interdisciplinary team participation, admissions, and discharges, internal and external transfers, delegating to ancillary staff, electronic medical record documentation, patient transport, and emergency response procedures.
Minimum Qualifications
Requirements:
Staff shall have BLS certified, and at least three (3) years of experience in acute care or other areas. The personnel assigned by the Contractor to perform the services covered by this task order shall possess an unrestricted license to practice medicine in one of the States, Territories, or of the United States or the District of Columbia.
