The Float Patient Care Coordinator is a support position for the Case Management Department and works under the direction of the Manager of Targeted Case Management. The Float does not have an assigned patient caseload; however, the position supports Acute, Residential and Resource Patient Care Coordinators with coverage needs. The Float Patient Care Coordinator provides skills coaching to patients in a group setting at least 4 times weekly and participates in 1:1 care coordination service.
The Float exemplifies proper Patient Care Coordinator process to include coordinated care to patients by developing, monitoring, and coordinating interdisciplinary care in conjunction with the treatment team. The Float Patient Care Coordinator provides effective leadership and advocacy in building relationships between teams, families, and other community professionals. The Float Patient Care Coordinator is required to complete their own billing and note documentation to comply with hospital and governing agency standards.
This position supports Acute and Residential Unit and Resource Patient Care Coordinators to identify, coordinates mental health and supportive services. The Float Patient Care Coordinator assists families in navigating community services and systems and can help in the application and referral process.

- Post-secondary education or Bachelor’s degree in a human services field strongly preferred.
- At least 3 year of experience serving people with mental illness is required.
- 5 years of experience in case management or patient care in a healthcare or social services delivery system preferred. 1 Year experience in a patient care setting.
