Summary
As a Case Manager at Arc of Maui County, you will be instrumental in coordinating care as part of the Interdisciplinary Team and providing support for 10 individuals with disabilities. This role is vital in ensuring that individuals receive the necessary resources and services to enhance their quality of life while fostering independence and self-determination.
Responsibilities
- Assess individual needs and develop individualized program plans to meet those needs.
- Coordinate with various service providers to ensure comprehensive care for individuals.
- Monitor individual's progress and adjust service plans as necessary to achieve desired outcomes.
- Provide crisis management support to individuals and families as needed.
- Facilitate communication between individuals, families, and service providers to ensure effective collaboration.
- Educate individuals and families about available resources and services.
- Maintain accurate records of individual interactions, progress, and service delivery.
- Advocate for individuals rights and access to necessary services.
Qualifications
- Bachelor's degree in Human Service related field.
- One year experience working with individuals with Intellectual/Developmental disabilities.
- Experience in social work or case management is highly desirable.
- Strong crisis management skills to effectively handle challenging situations.
- Knowledge of behavioral health practices and principles.
- Ability to work collaboratively with diverse populations and service providers.
- Excellent communication skills, both verbal and written.
- Proficient in Microsoft Office programs.

Requirements
