Job Summary:

In this role, you will play a critical role in ensuring accurate and efficient management of patient medical records and referral follow-ups. Responsibilities include obtaining and linking medical records to patient charts, verifying insurance and claim eligibility, documenting appointment details, and supporting the Pharmacy 340B team. This role also requires excellent communication skills for interacting with patients, providers, and other healthcare team members.

Key Responsibilities:
- Obtain and organize medical records from patient referrals and external appointments
- Accurately document and link records to patients' medical charts
- Verify insurance information and claim eligibility as required
- Support the Pharmacy 340B team with appointment documentation and program tasks
- Answer phones and assist with inquiries related to medical records and referrals
- Collaborate with healthcare providers and staff to ensure seamless care coordination
- Maintain confidentiality and compliance with all regulations regarding patient information
- Utilize electronic medical record systems efficiently and accurately
- Participate in team meetings, training sessions, and other professional development opportunities
- Perform additional clerical and administrative duties as assigned
