POSITION DESCRIPTION
TITLE : Case Aide
JOB CLASSIFICATION : Non-Exempt Full-Time
ORGANIZATIONAL UNIT : Catholic Social Services, Women of Hope-Lombard
Housing & Homeless Services Division
SUPERVISION : Direction from Program Supervisor
MAJOR RESPONSIBILITIES :
The primary responsibility of the Case Aide is to provide supportive services and resources to residents to ensure stability and strong life skills. Work collaboratively with Life Skills Specialist. Ensure all residents receive proper supports and certify services are delivered in accordance with state regulations and DBHIDS (Department of Behavioral Health and Intellectual Disability Services).
PRINCIPLE DUTIES AND RESPONSIBILITIES :
- Engage residents and establish positive rapport.
- Meet with residents on a weekly basis.
- Assist residents with skills building, recovery / life goal setting, problem solving, and utilize self-help recovery tools.
- Work collaboratively with other community organizations as directed.
- Complete and submit all time sensitive paperwork required by City of Philadelphia, and DBHIDS as outlined by in program policies.
- Complete assessments and referrals for outside housing providers.
- Accompany residents in the community as needed to accomplish identified goals and establish community support services including but not limited to doctor appts and housing options.
- Maintain accurate records of resident progress.
- Provide support and work collaboratively to the social worker and program supervisor.
- Assist residents in investigating possible programs (job training, dual diagnosis group meeting, education, etc.) in preparation for transition to permanent housing.
- Assist residents in understanding proper medication usage and dispensing.
- Monitor and track resident medication distribution when needed.
- Assist with medication distribution when needed.
- Complete and manage data entry for residents’ in software systems for CSS and DBHIDS.
- Coordinate and participate in monthly resident activities.
- Participate in monthly staff meetings, weekly supervision, training and meetings required by both CSS and funders.
- Other tasks and duties as assigned.
