Responsibilities
1. Admissions Management
o Oversee the entire admissions process from receipt of referral to admission.
§ Will ensure timely response to referral questions.
o Schedule and conduct tours of the facility for prospective residents and their families.
o Evaluate potential residents for eligibility based on clinical and financial criteria.
o Responsible for the organization and maintenance of the Facility Resident Bed Board.
2. Financial Eligibility and Background Checks
o Perform thorough financial eligibility checks to ensure prospective residents meet the facility’s requirements.
o Conduct background checks on all potential residents to comply with regulatory standards.
3. Room Readiness
o Coordinate with the housekeeping and maintenance teams to ensure that rooms are clean, well-maintained, and ready for new residents.
o Inspect and audit rooms prior to admission to confirm readiness.
4. Contract Management
o Ensure that all admission contracts and necessary paperwork are completed and signed by the appropriate parties.
o Maintain accurate and up-to-date records of all admissions documents.
5. Follow-Up with New Admissions and Facility Residents
o Follow up with new residents and their family post-admission to address any concerns and ensure satisfaction.
§ Ensure this is done in scheduled intervals.
o Act as a liaison between residents, families, and facility staff to facilitate smooth transitions.
6. Referral Tracking
o Maintain and update the referral tracker in real time to monitor and manage incoming referrals.
7. EHR and Census
o Admit, transfer, and discharge residents in PointClickCare
o Send daily census and forecast
8. Oversee Reception Department
o Responsible for daily reception schedule and ensuring coverage.
Qualifications
- Education: Bachelor’s degree in healthcare administration, business administration, or a related field.
- Experience: Minimum of 3-5 years of experience in admissions, preferably in a skilled nursing facility or healthcare setting.
- Skills: Strong organizational, communication, and interpersonal skills. Proficiency in Microsoft Office and experience with electronic health record (EHR) systems.
- Knowledge: Familiarity with Medicare, Medicaid, and private insurance processes. Understanding of state and federal regulations related to skilled nursing facility admissions.
