- Seeking a board certified Internal Medicine Nurse Practitioner / Physician Assistant
- Diagnose and treat patients with acute and chronic conditions as well as serving as a consultant and educator.
- Performing patient history and physical examinations, diagnostic testing, test interpretation, treatment, patient follow up and education on managing acute and chronic health problems.
- The APP promotes collaboration between the multiple disciplines to ensure cost effective and quality patient care.
- The APP will make referrals to physicians and other healthcare providers, document assessment, management, and education clearly and concisely within the Electronic Health Record (EHR).- The APP will dictate discharge summaries, writes prescriptions, explains discharge instructions, and confirms follow-up appointments, as well as monitor, record and report systems or changes in patients condition.
