- Full-time, permanent- This employed position has a competitive salary and a comprehensive benefits package.- The NP is responsible to work in collaboration with patients, their families and other caregivers, the patients primary care physician, and other specialists as appropriate, in an active practice to deliver medical management of symptoms related to advanced illness.
The work environment will include time seeing patients in their home.
The Practitioner functions in a manner which maintains the confidentiality and privacy of each patient, promotes a positive relationship with all health team members and supports the policies and procedures of the hospital and medical group.
Essential Functions :
Knowledge / Skills / Abilities :
Must be able to exercise sound clinical judgment.
Demonstrated experience in clinical practice.
Maintain a broad base of technical knowledge and skills to perform all assigned clinical / administrative duties.
Knowledge of palliative care and hospice regulations, clinical practice, end of life care services, and advance care planning.
Demonstrate excellent teaching skills to explain medical information to the patient and family.
Must be able to coordinate and communicate effectively with colleagues, managers, and other medical staff.
Develop a plan of care that is individual to each patient and ensure that the primary care physician and applicable specialists receive regular updates.
