- Document and communicate standard of work for social work across the organization.
- Collaborate with Data Engineering Social Worker metrics and incentive compensation structure.
- Participate in Social Worker recruiting and interviewing as requested by Directors of Market Operations.
- Refine and execute new social work colleague onboarding and continue education activities in collaboration with market Directors of Operations.
- Design and implement Social Worker coverage for new markets/open positions including supporting multi-state licensure and distribution of caseloads.
- Organize and lead monthly Social Worker meeting.
- Lead as Subject Matter Expert on SDOH CMS STAR measures and collaborate with the central Quality team.
- Meet regularly with the Director of Care Management and the Director of Member Financial Counseling on organizational workflows and referral processes including Medicaid and Cost Share Assistance applications.
- Plan and perform Social Worker QA activities including reviewing market support packages and resources, social worker documentation and compliance with HIPAA policies.
- Collaborate with managed care plans and disseminate market resources when available.
- Identify methods to improve ArchWell Health's social work approach based on feedback and regularly conducted surveys.
- Support Advance Directive initiatives within the organization.
- Report out to senior leadership as needed.
- Experience supporting patients with need for social services
- Experience with screening, assessment, and planning for common social services needs
- Working knowledge of differential diagnosis of common mental health conditions
- Strong interpersonal communication skills with exceptional active listening abilities
- Highly empathetic, non-judgmental, and open-minded
- Experience in a collaborative team environment
- Fully licensed LMSW in the current State of practice preferred
- Master’s degree in social work
- 1 years' experience in clinical social work
- A problem-solving orientation and a flexible and positive attitude
