Duties/Responsibilities:
- Developrelationshipsandcollaboratewiththeprimarycareteamstoconductsocialservicesassessmentstodeterminetheappropriateneedsforeachmember
- Plan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriers
- Navigatemanagedcareplansforcommunityservicesandprograms
- Casemanagementforsocialandbehavioralcaretoallowmemberstoself-managehealthandsocialservicesupport
- Accurateandtimelydocumentationofpatientencountersandsessionsinallclinicalmanagementsystems
- Proactivelyidentify methods toimprove ArchWellHealth's approachbased onfeedbackandregularlyconductedsurveys
- Support advance care transitions with members and their families
- Assist members with access to state-based prescription programs and other benefits
RequiredSkills/Abilities:
- Experiencesupportingpatientswithneedforsocialservices
- Experiencewithscreening,assessment,andplanningforcommonsocialservicesneeds
- Workingknowledgeofdifferentialdiagnosisofcommonmentalhealthconditions
- Stronginterpersonalcommunicationskillswithexceptionalactivelisteningabilities

EducationandExperience:
- FullylicensedLMSWinthedesiredStateofpractice
