Brief Summary:

Responsible for the administrative operation of the Samuel Simmonds Memorial Hospital Pharmacy, maintenance of all pharmaceutical policies, procedures and quality control practices, and for the management and supervision of Pharmacy staff. Helps to ensure that the pharmacy provides optimal services; facilitates safe medication use; meets all legal, accreditation, and certification requirements; and complies with all applicable policies, codes, and standards of the organization.

Education:

  • A Bachelor's degree in Pharmacy required; an advanced degree (Pharm D. or M.S.) preferred.
  • Four or more years of experience in hospital and retail setting required, with two years in a supervisory capacity.
  • Basic Life Support (BLS) competency required within 90 days.
  • Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) competency preferred.
  • Must have valid unrestricted license to practice pharmacy and to dispense controlled substances within the United States.

Principle Duties/ Responsibilities

  • Conceptualize, design, demonstrate, and document pharmaceutical care. Maintain and recommend changes to the SSMH medical staff approved formulary.
  • Develop, implement, and evaluate treatment guidelines that address cost-effective and appropriate drug therapy.
  • Assists with the development of policies and procedures that promote safe, appropriate, and cost-effective medication use.
  • Provide accurate, safe, efficient, and cost-effective pharmaceutical care, either directly or indirectly through the supervised efforts of support staff, to the patients of Samuel Simmonds Memorial Hospital and retail consumers.
  • Act as specialist in an assigned area of expertise, engage in consumer education, and provide clinical practice direction to other staff members.
  • Participate in interdisciplinary groups to enhance patient care.
  • Develop, evaluate, report, and otherwise participate in Continuous Quality Improvement, Drug Utilization Evaluation, Adverse Drug Reaction reporting, Medication Use, and other projects as assigned.
  • Provide, coordinate, and evaluate staff development opportunities for practicing pharmacists. Evaluate clinical skills of pharmacists and pharmacy technicians and provide performance review.
  • Provide accurate, efficient and appropriate pharmaceutical distribution services to patients by properly interpreting physicians' drug orders, entering orders into the department's computer system, and dispensing the appropriate medications utilizing the accepted departmental programs for these activities.
  • Identify, resolve, and prevent potential and actual drug-related problems by obtaining and evaluating medical data including patient history, laboratory data, and review patient profiles for drug interactions, allergies, and contraindications, adverse drug reactions to determine and implement optimal drug therapy.
  • Communicate directly with physicians, nurses, and other health care providers to relate information regarding patient drug therapy.
  • Communicate and document drug information given to patients and/or patient representatives.
  • Monitor all patient drug therapy for cost effectiveness and communicate all possible cost saving recommendations to patient's physician(s).
  • Maintain and update personal drug knowledge database to ensure accurate provision of pharmaceutical care for general disease states as well as specialized knowledge in the assigned area of expertise; maintain advanced practice base through the avenues of publications, presentations, educational boards, and professional consultation, association leadership, and certification.
  • Maintain current knowledge of state and federal regulations and statutes regarding the practice of pharmacy and the distribution of approved drugs, controlled substances and investigational agents; conduct personal practice in accordance with those requirements.
  • Supervise support personnel according to state law to ensure accurate preparation and dispensing of pharmaceuticals. Assume total responsibility for the delivery of pharmaceutical and clinical services provided by any personnel under the position's direct supervision.
  • Supervises medication storage and preparation areas throughout the facility; establishes specifications for the safe and secure storage of medications, chemicals, and biologics.
  • Facilitates the implementation and maintenance of safe medication-use process throughout the organization.
