The ideal candidate is a self-starter, can manage their own schedule, is dependable, responsive to patient needs, communicates well with the team, and can show compassion and empathy in all interactions with patients and their loved ones.

This is a full-time position. The RN case manager will collaborate with the interdisciplinary team to create and implement care plans that meet patients and their loved ones' physical, spiritual, emotional, and psychosocial needs. The RN case manager will act as a role model, providing leadership and support to the hospice staff.

The Hospice RN is identified as the Registered Nurse Case Manager who oversees the patient care and coordination of care and ensures the ongoing assessment of each patient and their loved ones' needs are implemented. The Registered Nurse is an interdisciplinary team member who works collaboratively with the Medical Director, Social Worker, and Spiritual Care Coordinator to create individual care plans.

The Hospice RN has autonomy over scheduled visits and travels locally to care for patients in their house, assisted living, or nursing facility.

Local travel to nursing facilities and the home setting is necessary.

RESPONSIBILITIES

The RN's leadership supports professional development and the excellent performance of each team member.

  • The RN must be able to work independently and make accurate and, at times, quick judgments.
  • Ability to supervise others appropriately.
  • Ability to respond appropriately to patient needs, including uncontrolled symptoms, caregiver questions, and pain management outside of a hospital setting.
  • Must be able to accept and care for patients from different social, racial, cultural, and religious backgrounds.
  • Understanding of or willingness to learn about the Hospice philosophy and principles of death/dying.
  • The Hospice RN must be passionate about serving patients and their loved ones on their hospice journey.
  • Demonstrate leadership abilities and must follow Arcturus HealthCare's code of conduct policy.
  • Must introduce yourself and communicate your role to the patient/family/caregiver, physicians, nursing facility staff, and other referral sources.
  • Must be flexible, innovative, and have excellent interpersonal skills.
  • Supporting patients and their loved ones on their hospice journey can be emotionally taxing. Great hospice RNs develop excellent coping mechanisms and emotional maturity. They depend on the support of their teammates and leadership to manage stress and avoid burnout.

REQUIREMENTS

  • Registered Nurse licensed in the state of Georgia
  • CHPN (Certified Hospice and Palliative Nurse) certification is preferred.
  • CPR Certification
  • Minimum of one-year general nursing preferred with current Hospice, home health, medical, surgical, or critical care experience. Experience with pain and symptom management interventions preferred.
  • Valid driver's license and reliable transportation that is insured in accordance with state and/or Hand in Hand Hospice's requirements.
  • Demonstrate excellent observation, assessment, nursing judgment, and communication skills. Ability to provide accurate written documentation by the end of shift.
  • Self-directed with the ability to work with little supervision.
