What are the responsibilities and job description for the Hospice Admission Nurse position at Arcy Hospice?
Arcy Hospice is a locally owned and operated hospice agency based in Flower Mound, TX. We primarily have patients in Collin, Dallas, Denton, Tarrant and Cooke counties but also serve patients in other counties in the DFW area as well.
Primary functions of this position are:
* Perform assessment/evaluation of potential patients to determine if they meet hospice criteria
* Register/admit patients onto hospice services
* Initiate plan of care
* Other administrative duties as assigned by the Clinical Managers.
Limited on-call responsibilities.
Ideal candidates will have prior experience with hospice and/or in a senior care setting.
Qualifications/Requirements:
* Possess understanding of hospice program and benefits
* Current and valid state nursing license
* Customer service focused
* Dependable
* Flexible
* Ability to work in an interdisciplinary setting
* Self-directed with the ability to work with little supervision
* Excellent communication skills
* Satisfactory criminal background check
* Satisfactory references from employers and/or professional peers
* Compliance with accepted professional standards and practices
* Valid driver's license and automobile that is insured in accordance with state requirements and is in good working order for travel to and from patient locations
* Minimum age - 21
