ResponsibilitiesThe RN Case Manager assesses the social, psychological, cultural, environmental and financial situations concerning the patient and family. He / she is a patient / family advocate, promoting rights and dignity and striving to involve the patient / family in aspects of care as appropriate. The RN Case Manager also facilitates the acute, rehabilitative, and long-term discharge processes to provide for an optimal age-specific continuum of care for the populations listed below. Accountable for the care of patients assigned to them in accordance with the Nurse Practice Act and Policies and Procedures of BSA QualificationsJob Requirements : Graduate of Accredited School of NursingCurrent License as RN in State of TexasBasic Life Support CertificationPreferred Job Requirements : Registered Nurse with two years of experience, preferable in a hospital setting
