Position Summary:
We are seeking a compassionate and dedicated Care Associate to provide essential care and support to patients in their homes in a retirement community. As a Care Associate, you will assist individuals with daily living activities, ensuring their comfort, safety, and well-being while adhering to the prescribed care plans. The ideal candidate is reliable, professional, and empathetic, with a passion for helping others maintain their independence in a home setting.
Key Responsibilities:
- Follow Care Plans: Adhere to the patient's assigned plan of care, reporting any changes or unusual conditions to the staff nurse promptly.
- Personal Care Assistance: Help patients with personal care tasks, including grooming, bathing, dressing, and maintaining hygiene.
- Household Support: Assist with light housekeeping duties such as laundry, shopping for groceries and household items, preparing and serving snacks, and supporting patients with eating as needed. Act as escort to/from dining hall or other activities on the property.
- Companion Support: Assist with pet care services on behalf of patient such as dog walking, emptying cat litter boxes, feeding pet, etc. Retrieve and review mail with patient, sit and read aloud with patient, keep patient company during dining times or on walks around property.
- Mobility Assistance: Aid patients in transferring, moving in and out of bed or chairs, walking, and other mobility tasks to ensure their safety and comfort.
- Emergency Support: Know when and how to seek emergency assistance if required to ensure the patient's safety and well-being.
- Comfort and Safety: Provide attentive care during your shifts, ensuring patient comfort and security when caregivers are absent.
- Professional Development: Attend in-service meetings and training sessions to maintain up-to-date knowledge and skills.
- Professional Conduct: Maintain a positive, professional attitude and project a caring image, wearing the company-issued ID badge at all times.
- Technology Use: Utilize applications like Paylocity, Authenticare, MatrixCare, HHAExchange, and Carebridge as required for recordkeeping and communication.
- Other Duties: Perform additional responsibilities as assigned by supervisors or management to meet the needs of the patient and team.

Requirements
- Minimum of a High School Diploma or equivalent.
- Must possess a valid driver’s license.
- Reliable transportation and proof of state required vehicle insurance.
- Current negative TB skin test or state health card.
- Completion of 75 hour Home Health Aide training class.
- Certified in CPR/First Aid.
- Smart phone with acceptable internet capabilities and data limits required to perform certain functions of position.
- Must pass background check and drug screening.
