Description
Responsible for assisting the Physical Therapist to improve mobility, relieve pain, prevent or limit permanent physical disability and promote overall fitness and wellness.
- Provide therapy services consistent with the patient’s approved plan of care to relieve pain, develop and restore function and maintain maximum performance.
- Document patient care observations, interventions and evaluation on the day services are rendered. Assist Physical Therapist with the preparation of patient progress notes.
- Confer with Physical Therapist to modify existing physical therapy treatment plans and training needs to achieve maximum results.
- Observe patients during treatments to compile and evaluate data on their responses and progress and provide results to physical therapist in person or through progress notes.
- Utilize varied solutions to educate and promote the health and independence of the patient which includes instruction in the use of prosthetic and orthotic devices and durable medical equipment.
- Assess, monitor, document and report progress of patient’s health and condition using required documents, reports and established timelines.
- Attend or conduct continuing education courses, seminars, or in-service activities.
- Communicate with or instruct caregivers on patient therapeutic activities or treatment plans.
Requirements
- Associate’s degree for Physical Therapist Assistant.
- Current state Physical Therapist Assistant (PTA) licensure.
- 6 months home health care experience preferred.
- Valid driver’s license.
- Current CPR certificate.
- Good organizational and communication skills.
