What are the responsibilities and job description for the Physical Therapist, Home Health position at Argus Home Healthcare?
We are looking for a PT to do in home visits in the Denver area! We services adult and geriatric clients on a per visit basis. One year prior clinical experience as a licensed Physical Therapist is preferred. As a home care Physical Therapist, you will be an integral member of the team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
- Summary Responsibilities
- Evaluation; initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate times
- Developing and revising plan of care
- Coordination of services in consultation with home health staff
- Discharge evaluation and planning
- Documentation of all visits
- Supervision of therapy assistants as appropriate
