About the Role
This position is ideal for an experienced nurse who wants to remain clinically involved in patient care without being a bedside nurse. The successful candidate will be organized, efficient, and able to manage their own work with autonomy.
Key Responsibilities
- Completes the Minimum Data Set (MDS), Care Assessment Activities (CAAs) and care plans within regulated time frames.
- Assesses residents through physical assessment, interview, and chart review.
- Discusses resident care needs with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff.
- Reviews information from hospitals, consultants, and outside agencies, and uses this information in the completion of assessments and care planning.
- Coordinates, identifies, and/or initiates significant changes to MDS care plans.
- Maintains current knowledge of the American Association of Nursing Assessment Coordinators (AANAC) requirements.
Requirements
- Registered Nurse (RN) license.
- AANC certification (RAC-CT) is preferred.
- Knowledge of the Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS) principles is strongly preferred.
- Experience as an MDS Nurse is required.
