Job Type Full-timeDescriptionASA Case Management SupervisorGrand Junction, COOne Full-Time Position. 40 hours per week.Monday through Friday, traditional hours.Hiring Range : $50,000 to $54,500 Annually, depending upon education and experience.This position will remain open until filled.Minimum Requirements Required :

  • Bachelor's degree or higher in social or behavioral sciences.
  • Two years Case Management experience in I / DD population.Preferred :
  • One-year direct experience in the Mental Health field.
  • Two years supervisory experience in a Human Services field.

  • And committed to a Culture of Awesomeness!Position SummaryThe Case Management Supervisor is responsible for providing support, direction, and oversight to include ensuring compliance with I / DD regulations and person-centered care. Ariel Clinical Services MissionTo provide safe, nurturing and supportive environments for children, families, and adults so that they may have the opportunities to develop their strengths, maximize their potentials and fully participate in society.Essential Duties and Responsibilities1. Oversight of case management training, supervision, and support to employees and independent contractors.2. Conduct interviews, hiring, performance appraisals, prepare and / or recommend disciplinary action of all employees supervised.3. Mentor case managers regarding creating and expanding program knowledge, resources, and opportunities.4. Assist Program Manager / Director in appropriate placement of adults into Ariel residential services.5. Actively participate in program evaluation, ensure program quality assurance.6. Represent Ariel and maintain a positive relationship by acting as a liaison to local human service agencies and state level agencies, as required.7. All duties contained in the Case Manager Position description.8. Ensure individual files are audit ready and in compliance with Volume 8 rules and regulations.9. Responsible for On Call or Back up after-hours program emergencies.10. Adhere to the accounting calendar and approve employee timecards.Additional Duties and Responsibilities 1. Must work closely with other Ariel Programs and offices.2. Performs other duties as assigned.3. Assist Program Manager with developing and implementing new and existing programs. RequirementsRequired Knowledge, Skills, and Abilities1. The ability to communicate respectfully and effectively in both individual and group settings.2. Ability to be flexible, and able to change direction rapidly with a positive attitude.3. Ability to work independently, prioritize workload, complete a wide variety of tasks in a timely and accurate fashion and direct others, as needed.4. Ability to deescalate aggressive behaviors without means of restraint.5. Basic computer knowledge.6. Be available throughout a 24-hour period on an "on-call" basis.7. Strong knowledge of adult services agency programs and community resources.8. Exercise independent, ethical judgment within the scope and boundaries of the department, the Agency's Standard Operating Procedures, and Federal, State and Local Rules and Regulations regarding department operations.9. Ability to organize and present case material in a straightforward and concise written manner.10. Ability to demonstrate regular attendance and to be a reliable and supportive team member.11. Ability to always represent Ariel in a professional and respectful manner.Education and ExperienceRequired :
  • Bachelor's degree or higher in social or behavioral sciences.
  • Two years Case Management experience in I / DD population.Preferred :
  • One-year direct experience in the Mental Health field.
  • Two years supervisory experience in a Human Services field.
  • Experience coordinating and collaborating services and program funding. Supervisory Responsibilities
  • Directly supervise Case Managers.Certifications / Licenses
  • Valid Regular Driver's License, with acceptable driving record.
  • CPR, First Aid - before contact with persons receiving services (PRS).
  • CPI - within 60 days of hire.
  • Informed Supervision -deviant sexual behaviors training - within 60 days of hire, may not be left unsupervised with PRS until this training is completed.
  • QMAP - (Medical Administration Certification) within 60 days of hire, may not provide medication to PRS without this certification.
  • BSIT
  • Patient-Centered Care training
  • Client Specific Training of individuals on caseload.Work Environment1. This position requires that the incumbent be capable of bending, sitting, stretching, and / or reaching regularly for periods up to two hours without a break. 2. This position requires that the incumbent have adequate hand and finger dexterity to properly perform the functions of the position.3. This position requires lifting regularly up to 10 pounds and occasionally up to 40 pounds.4. This position requires the ability to hear, see, smell, and distinguish temperature, at a level which is adequate to ensure client and staff health and safety. Adaptive devices are acceptable.5. This position will require daily driving of up to one hour per day and occasionally up to eight hours per day. Exposure to typical climate-controlled office environment when in the office.6. Exposure to all climates and weather conditions while working in the field.7. At times, physical interventions may be required.8. Exposure to blood borne pathogens and infectious disease is possible but limited.9. Exposure to hazardous chemicals is possible but limited.Required Background Checks & Documentation1.
