Case Manager for Adult Services

One Full-Time Position. 40 hours per week.

Monday through Friday, traditional hours. On-call rotation.

Hiring Range: $20.16 - $21.97 / hr., depending upon education and experience.

This position will remain open until filled.

Minimum Requirements

Required:

  • Bachelor’s degree in social or behavioral sciences, or a combination of four years’ experience, and formal education in a Human Services field.

Preferred:

  • Experience with evidence based individual behavioral supports and services.
  • Minimum of two years working with I/DD population.
  • One year experience mental health experience.

Position Summary

The ASA Case Manager is responsible for providing person-centered care coordination for individuals with developmental and intellectual disabilities to promote a high quality of life.

Ariel Clinical Services Mission

To provide safe, nurturing, and supportive environments for children, families, and adults so that they may have the opportunities to develop their strengths, maximize their potentials and fully participate in society.

Essential Duties and Responsibilities

1. Establish, build, and maintain rapport with individuals, independent contractors, and other members of the multidisciplinary team in order that optimal information gathering, and collective decision making may occur. These duties must be carried out in a manner that respects the rights and dignity of individuals, is driven by individual choice and promotes community inclusion.

2. Provide case management for individuals. Work closely with, and act as the liaison between collaborating agencies/multi-disciplinary team.

3. Develop, monitor, and document mandated services and programming, and ensure individual satisfaction. Ensure individual files are audit ready and in compliance with Volume 8 rules and regulations.

4. Understand and participate in all relevant assessments to ensure appropriate level of care for individuals.

5. Facilitate home visits and meetings to assess the needs and progress of individuals and intervene as appropriate. Provide mediation as needed.

6. In a timely manner, effectively and appropriately communicate potential risks and/or threats to an individual or program to Ariel supervisors and collaborating agencies.

7. Participate in mandatory rotation for “on call” to support our individuals.

8. Participate in ongoing program evaluation to ensure program effectiveness.

1. Must work closely with other Ariel Programs and offices.

2. Performs other duties as assigned.

Requirements:

Required Knowledge, Skills and Abilities

1. The ability to communicate respectfully and effectively in both individual and group settings.

2. Ability to be flexible, and able to change direction rapidly with a positive attitude.

3. Ability to work independently, prioritize workload, complete a wide variety of tasks in a timely and accurate fashion and direct others, as needed.

4. Basic computer knowledge.

5. Exercise independent, ethical judgment within the scope and boundaries of the department, the Agency’s Standard Operating Procedures, and Federal, State and Local Rules and Regulations.

6. Ability to organize and present case material in a straightforward and concise written manner.

7. Ability to provide reliable, insured transportation for self and others through the community served.

8. Ability to be a reliable and supportive team member.

9. Ability to always represent Ariel in a professional and respectful manner.

10. Knowledge of community resources (social, public agency, and information) to assist persons receiving services (PRS) in accessing these resources.

11. Ability to demonstrate consistent and reliable attendance as scheduled and requested.

Education and Experience

Required:

  • Bachelor’s degree in social or behavioral sciences, or a combination of four years’ experience, and formal education in a Human Services field.

Preferred:

  • Experience with evidence based individual behavioral supports and services.
  • Minimum of two years working with I/DD population.
  • One year experience mental health experience.

Certifications/Licenses

  • Valid Regular Driver’s License, with acceptable driving record.
  • CPR, First Aid – before contact with persons receiving services.
  • CPI – within 60 days of hire.
  • Informed Supervision –deviant sexual behaviors training - within 60 days of hire, may not be left

unsupervised with PRS until this training is completed.

  • QMAP – (Medical Administration Certification) within 60 days of hire, may not provide medication to

PRS without this certification.

  • Client Specific Training of individuals on caseload.
  • BSIT
  • Person-Centered Training

Physical Demands

1. This position requires that the incumbent be capable of bending, sitting, stretching, and/or reaching regularly for periods up to two hours without a break.

2. This position requires that the incumbent have adequate hand and finger dexterity to properly perform the functions of the position.

3. This position requires lifting regularly up to 10 pounds and occasionally up to 40 pounds.

4. This position requires the ability to hear, see, smell, and distinguish temperature, at a level which is adequate to ensure persons receiving services and staff health and safety. Adaptive devices are acceptable.

5. This position will require daily driving of up to one hour per day and occasionally up to eight hours per day.

Work Environment

1. Exposure to typical climate-controlled office environment when in the office.

2. Exposure to all climates and weather conditions while working in the field.

3. Exposure to blood borne pathogens and infectious disease is possible but limited.

4. At times, physical interventions may be required.

5. Exposure to hazardous chemicals is possible but limited.
