ARISE is looking for a Consumer Directed Coordinator. The Consumer Directed Services (CDS) Coordinator provides the necessary supports to Consumers receiving Consumer Directed Personal Assistance and Consumer Directed In-Home Services for them to be able to manage their services

Providing Consumers and their Personal Assistant(s) (PAs) with an orientation to receiving Consumer Directed Personal Assistance (CDPA) and Consumer Directed In-Home Services (CDIHS). Specific to CDPA: o Assisting Consumers with the process of registering with the SFI. o Assisting PAs with the process of registering as a CDPAP PA with the SFI. o Informing Consumers of service authorization start date and parameters. o Monitoring EVV compliance by Consumers and PAs. o Following up directly with Consumers and PAs in situations where EVV utilization is a concern to identify barriers to compliance and working with the Consumer to develop and implement a plan to resolve or minimize the non-compliance. o Monitoring PA overtime and working with the Consumer to develop and implement a plan to minimize the use of overtime as appropriate without reducing necessary services to the Consumer. o Providing telephonic, electronic or in-office customer support to Consumers and PAs to answer questions about CDPAP. |
Specific to CDIHS, if applicable:
o Support Consumers and/or their Designated Representative (DR) to manage the Consumer’s plan of care including recruiting and hiring a sufficient number of PAs to provide authorized services;
training, supervising and scheduling each PA; terminating the PAs employment and ensuring that each PA competently and safely performs the personal care services that are included on the Consumer's plan of care.
o Provide support and guidance, as necessary, to the Consumer/DR regarding the hiring process; initial and annual training; oversight of accurate and timely service submissions; requested application/software training; issue resolution; etc.
o Meet with and educate potential Consumers and their support network about available services, assess needs, and make appropriate program recommendations.
o Schedule and conduct intakes and annual monitoring calls.
o Maintain regular communication with the entity authorizing services.
Monitoring, maintaining, and auditing program files to ensure compliance with regulatory requirements.
Attending and actively participating in inter-departmental meetings, meetings with funding representatives, and out-of-town networking conferences as needed.
Upholding codes of conduct and ethics, as well as all ARISE policies and procedures. Exercising sound judgment and maintaining confidentiality of protected health information and other sensitive data in accordance with ARISE and HIPAA policies.
