Senior Community Social Services Coordinator

Overview
AristaCare at Norwood Terrace is seeking a highly skilled and dedicated Social Worker to join our team in providing exceptional care and support services to our residents.

Job Summary: The successful candidate will be responsible for assisting in the planning, organization, and development of the Social Services Department. This includes ensuring that the medically related emotional and social needs of each resident are met and maintained on an individual basis.

Key Responsibilities:

• Collaborate with healthcare professionals to develop and implement personalized care plans.
• Conduct assessments to identify resident needs and provide recommendations for support services.
• Develop and maintain relationships with residents, family members, and external partners to ensure a seamless care experience.

Requirements:

• Bachelor's degree in Social Work or Human Services.
• Current certification as a Social Worker or Licensed Social Worker in the state of New Jersey.
• Experience working in a Skilled Nursing Facility or Assisted Living environment preferred.
