Who We Are Looking For
Arizona Autism is seeking an experienced Direct Care Worker to make a positive impact in the lives of our adult co-ed members living in a family-style environment. We are looking for individuals who possess a caring heart and a passion for serving our clients.
Responsibilities of the Residential Group Home Direct Care Worker include, but are not limited to:
ISP:
- Carry out and coordinate with GH Manager and GH Director for the implementation of the Individual Service Plans (ISP)
- Carry out and document submitted ISP and BTP goals.
- Monitor and advocate for new goals and past goals for the member to live an auspicious lifestyle.
Support:
- Facilitate professional interactions with both internal and external stakeholders.
- Provide support to individuals receiving services including assistance with daily activities such as meal preparation and planning; medication administration; and personal care.
- Properly track low volume medications for each consumer present in the facility.
- Facilitate community involvement by accompanying individuals on outings in the community or providing transportation to work, medical appointments (as needed), or other activities.
Financial:
- Provide receipts and financial documentation to GH Manager for consumer outings.
Provider Requirements
- Level One Fingerprint Clearance Card
- Driver’s License
Job Types: Full-time, Part-time
