The Department of Economic Security, Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to join our team as a QIDP Nurse Case Manager who support individuals in State Operated Services ICF / IID (Intermediate Care Facilities for the Intellectually Disabled) settings. This position functions as a QIDP (Qualified Intellectual Disability Professional) for these settings as required by state and federal ICF standards. DDD provides individuals with developmental disabilities, and their families, services and supports that are flexible, high quality, and member driven. These services provide individuals with opportunities to exercise their rights and responsibilities of independent decision-making and engagement in the community.
This position may offer the ability to work remotely, within Arizona, based upon the department's business needs and continual meeting of expected performance measures. The State of Arizona strives for a work culture that affords employees flexibility, autonomy, and trust. Across our many agencies, boards, and commissions, many State employees participate in the State's Remote Work Program and are able to work remotely in their homes, in offices, and in hoteling spaces. All work, including remote work, should be performed within Arizona unless an exception is properly authorized in advanced.
Applicants for this position cannot be licensed or certified by the DDD Office of Licensing Certification and Regulation, cannot have a principal interest in or have a spouse or close family with principal interest in a DDD Qualified Vendor Agreement, and cannot be employed or subcontracted by a DDD Qualified Vendor.
Job Duties :
Essential Duties and Responsibilities include but are not limited to :
- Coordinates and monitors long term care, physical health, and behavioral health services for members. Facilitates planning meetings, complete nursing assessments, active treatment observations, discharge planning, specialty care, arranging for medical equipment promptly addressing changes in condition, coordinating physical health and behavioral health appointments and follow up.
- Provides oversight relating to services the member receives and reports quality of care concerns promptly per DDD's policies and procedures. Develop rapport and collaborate with resident, families and guardians, residential and day program staff, and medical professionals.
- Complete ongoing comprehensive progress notes that analyze the member progress, regression, or maintenances of skills. Continuously evaluate the member needs for services & supports, conduct referrals when a need is identified and ensure follow up is completed on recommendations for services, equipment, therapies, or programs. Monitors to ensure adequate services & support are present to promote independence. Completes authorizations.
- Prepare a variety of reports and completes special projects. Attend staffing meetings, training and other meetings (including statewide travel). Confer with supervisor.
- Ensure residential and day program staff are trained on the needs of the residents. Conduct incidental training as needed to ensure staff follow Active Treatment program / ISP / therapeutic plans for each resident.
