Arizona Home Care, the largest locally owned Medicare Certified Home Health Agency in Phoenix, is looking for a skilled Occupational Therapist to join our team on a PRN basis in Anthem, AZ. Position Highlights:
- Pay: $90 per unit
- Work Type: PRN (as-needed basis)
- Location: Anthem, AZ
- 1-2 years of Home Health experience required
- Fluency in OASIS is required (KanTime EMR knowledge is helpful but not required)
Key Responsibilities:
- Conduct comprehensive assessments of patients’ needs in their home environment
- Develop and implement individualized treatment plans to enhance patients' ability to perform daily activities
- Provide skilled occupational therapy services, adjusting interventions as necessary to achieve patient goals
- Educate patients and caregivers on strategies to improve functional mobility and independence
- Ensure timely and accurate documentation in compliance with OASIS and regulatory standards
- Collaborate with a multidisciplinary team to deliver optimal care
- Maintain positive, professional relationships with patients, families, and colleagues
Requirements
Qualifications:
- Active Occupational Therapist license in Arizona
- 1-2 years of Home Health experience required
- Fluency in OASIS required (KanTime knowledge a plus)
- Arizona Level One Fingerprint Clearance Card required
- Valid Driver's License and car insurance card
- Current CPR and First Aid certification required
- Current TB test
- Two professional references required at time of interview
- Compassionate, patient-focused approach to care
