The Triage/Visit nurse is a valuable member of the hospice team and will be responsible for meeting the needs of patients outside normal business hours. The nurse is responsible for triaging calls, making home visits, ordering medications and supplies, directs hospice care and working as part of the interdisciplinary team to provide a patient-centered end-of-life care and symptom management to ensure quality care is provided and to meet the needs of individual and families within their homes and communities.
Weekday (Night Shift) Monday - Thursday 5:00 PM to 8:30 AM.
