We are looking for a Telephone Triage - Licensed Practical Nurse that focuses on bringing understanding and peace to patients and their families during what may seem like a confusing and challenging time.

Responsibilities :
In your job, you will have the ability to make a difference in a patients journey by :
- Address patient calls, portal messages, and respond to emails in a timely and professional manner.
- Ask open-ended questions while utilizing computerized algorithms / pathways to assist in the assessment of the patients symptoms and / or side effects with focus on addressing swiftly and appropriately.
- Determine if emergency care is needed collaborate with physicians via electronic, face to face or telephone communication to discuss patient care needs that are not able to be independently addressed using standard orders or practice protocols.
- Manage symptoms to prevent unnecessary emergency department and hospital utilization.
- Collaborate and partner with providers if the patient is able to be seen in an outpatient clinic setting to address symptoms.
- Document all conversations with patients to ensure an accurate and comprehensive medical record.
- Provide clear and concise instruction and education to patients over the phone providing educational documents as needed.
- Phone in prescriptions and make other patient calls as required or directed by physicians.
- Maintain patient files, records, and other information.
